Message	feeling you have been driving like this for a bit. I saw a similar failure once and the only idler that looked like that was the upper center. I have never checked the tq. of the bolt that holds the tensioner together, for that matter AFAIK there is no spec. listed for it. To me it looks like the tensioner was not installed correctly and was over tensioned.
